Job Summary:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our private duty and nursing care team. In this role, you will be responsible for delivering exceptional nursing care to our clients in their homes, providing specialized care that promotes healing and independence. Key Responsibilities:
1. Patient Care:
– Perform comprehensive assessments of patients’ health needs and create individualized care plans.
– Provide direct nursing care, including medication administration, wound care, IV therapy, and pain management.
– Monitor and document patients’ progress and communicate any changes in condition to the appropriate stakeholders.
– Educate patients and families on disease management, health maintenance, and safety protocols.
– Collaborate with other healthcare professionals to coordinate patient care.

 

2. Care Coordination:
– Work closely with physicians, therapists, and other healthcare team members to ensure seamless patient care.
– Ensure all patient care documentation is accurate and submitted promptly.
– Develop and implement discharge planning in collaboration with interdisciplinary teams.

 

3. Leadership and Training:
– Supervise and provide guidance to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
– Assist in training new nursing staff and ensure compliance with best practices.
– Advocate for patients’ rights and help them navigate healthcare systems.

 

4. Compliance and Safety:
– Maintain compliance with all federal, state, and local regulations, including HIPAA.
– Adhere to company policies and procedures, ensuring patient confidentiality and safety.
– Participate in quality improvement initiatives and safety drills.

 

Qualifications:
– Current Registered Nurse (RN) license in Michigan.
– Bachelor’s degree in Nursing (BSN) preferred.
– Minimum of 2 years of nursing experience, preferably in home health or private duty care.
– Strong clinical skills and knowledge of care practices.
– Ability to work independently and make decisions in a fast-paced environment.
– Exceptional communication and interpersonal skills.
– Proficiency in electronic health record (EHR) systems.
