Job Summary:<\/strong>
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our private duty and nursing care team. In this role, you will be responsible for delivering exceptional nursing care to our clients in their homes, providing specialized care that promotes healing and independence. Our ideal candidate is empathetic, has excellent clinical skills, and is committed to providing personalized care.<\/p>

Key Responsibilities:<\/strong>
1. Patient Care:
\u2013 Perform comprehensive assessments of patients\u2019 health needs and create individualized care plans.
\u2013 Provide direct nursing care, including medication administration, wound care, IV therapy, and pain management.
\u2013 Monitor and document patients\u2019 progress and communicate any changes in condition to the appropriate stakeholders.
\u2013 Educate patients and families on disease management, health maintenance, and safety protocols.
\u2013 Collaborate with other healthcare professionals to coordinate patient care.<\/p>

2. Care Coordination:
\u2013 Work closely with physicians, therapists, and other healthcare team members to ensure seamless patient care.
\u2013 Ensure all patient care documentation is accurate and submitted promptly.
\u2013 Develop and implement discharge planning in collaboration with interdisciplinary teams.<\/p>

3. Leadership and Training:
\u2013 Supervise and provide guidance to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
\u2013 Assist in training new nursing staff and ensure compliance with best practices.
\u2013 Advocate for patients\u2019 rights and help them navigate healthcare systems.<\/p>

4. Compliance and Safety:
\u2013 Maintain compliance with all federal, state, and local regulations, including HIPAA.
\u2013 Adhere to company policies and procedures, ensuring patient confidentiality and safety.
\u2013 Participate in quality improvement initiatives and safety drills.<\/p>

Qualifications:<\/strong>
\u2013 Current Registered Nurse (RN) license in Michigan.
\u2013 Bachelor\u2019s degree in Nursing (BSN) preferred.
\u2013 Minimum of 2 years of nursing experience, preferably in home health or private duty care.
\u2013 Strong clinical skills and knowledge of care practices.
\u2013 Ability to work independently and make decisions in a fast-paced environment.
\u2013 Exceptional communication and interpersonal skills.
